A Guide to Understanding Genetic Counseling Before IVF

April 29, 2025

For many individuals and couples pursuing IVF, navigating genetic testing can feel like stepping into unfamiliar, and sometimes intimidating, territory. Questions arise quickly, and emotions often follow closely behind. That’s where genetic counseling before IVF comes in: not to add pressure, but to provide information, clarity, and promote choice.

At Blue Sky Fertility, genetic counseling is led by Alex Widman, MS, who earned her master’s degree from Washington University School of Medicine in St. Louis. With both compassion and clinical expertise, she supports patients in understanding how their genetic background may impact their fertility journey. Widman empowers them to make informed decisions at every stage.

“What does a genetic counselor do in fertility care?”

In the fertility setting, genetic counselors help patients understand their genetic information and what it could mean for their reproductive health and future children. The goal is not to make decisions for patients, but to provide the information and support needed to navigate complex topics like carrier screening, embryo testing, and inherited conditions.

There is no, “one-size-fits-all,” formula for genetic counseling. A genetic counselor tailors their approach to meet each patient’s goals, values, and comfort level. At Blue Sky, we are dedicated to creating space for thoughtful and personalized choices.

Common Misconception: “Genetic screening and testing is mandatory.”

Genetic counselors can share professional recommendations and provide clarity on your genetic testing options. Part of the genetic counseling process includes sharing how genetic information can impact one’s own personal health and how this information can be used for family planning. With this information, patients can feel empowered to make their own decisions regarding testing and screening.

Ultimately, the choice to pursue genetic testing is a personal one, and genetic counseling can help guide patients towards the decision that feels best for themselves and their families.

For patients who pursue screening, genetic counseling can provide further clarity on what options may be available to them. For example, patients who have genetic carrier screening may learn that they may have a higher chance to have a child with a rare genetic condition. In these cases, the counselor’s role is to explain the risk, offer insight into the next steps, and support the patient in whatever path they choose to take.

When Do Patients Meet with a Genetic Counselor?

At Blue Sky Fertility, most patients are referred for genetic counseling after initial fertility consultations and diagnostic testing, but before starting fertility treatment. This timing ensures that patients are equipped with the knowledge and support they need to move forward confidently.

The counseling process aims to catch patients at a time where they have the most reproductive options available to them.

Meeting with patients prior to starting fertility treatment allows patients to consider all options which may include PGT (preimplantation genetic testing or embryo screening) or utilization of donor gametes (donor sperm, eggs, or embryos). Counseling at this time can serve as a critical point of reflection, as patients decide how best to move forward on their fertility journey.

Who Should Consider Genetic Counseling?

Anyone with abnormal or positive genetic testing results, a personal or family history of genetic conditions, or even general questions about testing options like PGT benefit from meeting with a genetic counselor.

Importantly, genetic counseling isn’t reserved for high-risk individuals—it’s for anyone seeking clarity, information, or peace of mind during a very personal and sometimes overwhelming process.

What About the Emotional Side?

Genetic information can elicit strong emotions including fear and anxiety about uncertainty. During your genetic counseling session at Blue Sky Fertility, patients will receive emotional support alongside education, helping patients process the information at their own pace.

For some, learning about one’s own genetic information can bring peace of mind. For others, it may feel overwhelming. In those cases, patients are encouraged to reflect on what kind of information they truly want to know. Counselors can also refer individuals to mental health providers or support groups, creating a network of care during what can be an isolating experience.

Genetic Counselors Don’t Make Decisions—They Support Them

Genetic counseling exists as a tool to share information, provide risk assessments, and empower decision-making. At Blue Sky Fertility, counseling is centered on guiding patients toward decisions that feel right for them, based on their values, goals, and unique circumstances.

Genetic counseling is about choice—not control. It’s a powerful resource for anyone looking to move forward with greater confidence, compassion, and clarity.
